A significant innovation in the treatment of hemorrhoids is the introduction of new methods (THD and HALL), which are based on a new treatment concept in which the hemorrhoids are not touched, but the arterial entrance is connected to a “shunt” and thereby stops the filling of the hemorrhoids with blood, which results in the permanent disappearance of complaints.
The K-centar Polyclinic from Zagreb was one of the first in the region to introduce THD or its full name Transanal Hemorrhoidal Dearterialization and thus made this state-of-the-art method of treating hemorrhoids available to our patients.
It is a minimally invasive method that permanently solves problems related to enlarged hemorrhoidal nodes without surgery, on an outpatient basis.
Hemorrhoids are one of the most common and most unpleasant health problems of modern man. Although as an indication they are common in younger people, it is believed that every other person after the age of fifty has problems with hemorrhoids, most often in the form of bleeding and pain, and sometimes more serious complications such as severe anemia, thrombosis and infection can occur.
Because people too often choose to suffer the symptoms without treating them adequately, complications occur. Often, when the so-called IV. degree of hemorrhoidal disease, the patient has nothing left but an extremely painful surgical procedure, which carries with it certain risks, a long recovery and potentially negative long-term consequences for the patient.

Various methods are used for the treatment itself, from creams and suppositories to surgical techniques. Unfortunately, the so-called home therapies in most cases do not give lasting results and the complaints usually return. On the other hand, operative methods can have serious side effects, and it is also known that the pain after surgery is extremely strong and lasts for several weeks.
WHAT ARE HEMORRHOIDS?
We are all born with small pads in the final part of the large intestine that consist of a tangle of veins and ligaments. It is believed that these pads have their function in defecation. The plexus of veins in the pads is connected directly to the surrounding arteries. We call this connection a “shunt” and it is normally closed. However, as long as it remains open, then the veins are filled with blood from the artery, which leads to the increase of the pads, their descent towards the anal opening, and then we speak of hemorrhoids. So, the main cause of hemorrhoids is an open “shunt”, and the appearance of hemorrhoids is influenced by several factors, from genetics to lifestyle and diet.
Precisely because this procedure solves the cause of the problem, the effect is permanent, and complications are extremely rare.
HOW IS THE THD METHOD PERFORMED?
The THD method is a minimally invasive procedure that is performed on an outpatient basis, there is no need for hospital treatment, and most patients can return to their normal daily activities after only a few days. With a special instrument, which has a Doppler probe, the open “shunt” is located and the arterial entrance to the shunt is ligated. So, there is no cutting or bleeding and the procedure can be done without anesthesia, but since the anal area is very sensitive, at the K-centar Polyclinic we perform the procedure under short intravenous anesthesia or potentiated local analgesia.
By tying off the arterial entrance to the “shunt”, we prevented the flow of blood, which will lead to the reduction of the hemorrhoidal pads to their normal size and the cessation of symptoms within a few weeks after the procedure. During this period, it is very important to maintain regular and soft stools, which is why we recommend patients to drink 1 tablespoon of paraffin oil every evening for a month. Paraffin oil is not resorbed, but passes through the intestines, “carrying” the stool with it and lubricating the intestine, which facilitates emptying. Since it is not resorbed, there is no danger of harmful effects.
Given that the THD procedure does not touch the hemorrhoids, but by tying off the arterial entrance to the “shunt” prevents the filling of the hemorrhoids, which is the main cause of hemorrhoids, it takes several weeks for the hemorrhoids to “empty” and reduce to their normal size, which makes them disappear. all symptoms. Precisely because this procedure solves the cause of the problem, the effect is permanent, and complications are extremely rare.
According to previous experience, the percentage of recurrence after THD procedure is below 3%. A big advantage is the fact that the pain after the procedure is incomparably less and lasts significantly less than after surgery.
WHAT IS THE PREPARATION PROCEDURE FOR THE THD PROCEDURE?
Another advantage of this procedure is that no special prior preparation or treatment is required, as the examination is done immediately before the procedure and is part of the procedure. All you have to do is contact the K-centar Polyclinic and make an appointment for the procedure. Based on the examination and discussion with the patient carried out by the board of specialist doctors – the surgeon and the anesthesiologist conducting the treatment, if it is indicated, the patient can continue with the treatment on spot. After the procedure itslef, there is no need for a longer stay in the clinic, and with the recommendations, the patient can go home. Apart from maintaining regular and soft stools and analgesics in case of pain, there is no need for other post-operative treatments.
